All participants who attend programs, rent our facility or volunteer at Easter Seals Camp Horizon are required to complete a waiver form and bring it with them when they arrive on-site. Participants 18 and older must complete the Adult Waiver and participants under the age of 18 must complete the Child Waiver ensuring that it is dated, signed and witnessed appropriately.
